CDT partners with people and organizations in the community to raise awareness of the importance of donation. If you would like to host a speaker for your office, school, faith community, social service group or other organization please fill out the form below. You can also download the pdf. file and mail or fax the completed form to CDT. Please make sure to fill out all of the information, a CDT staff member will be contacting you.
